Contact us for more general information or to schedule an intake with a therapist via our email: clinic.admin@intune-d-t.com, or by phone: 617-764-1997.
Once an intake is scheduled, our clinic administrator will email you a registration packet including a referral questionnaire, consent form and schedule of rates. These forms need to be submitted before the initial intake.
